Under the direction of the department manager, the Medical Assistant will be assisting the team in referral management for High-Risk patients and helping to assist with patient needs.

Requirements Responsibilities/Day in the Life: Receive new patient referrals from clients Create charts for all new patients Input medical information in new chart Help Care Coordinators with patient needs as applicable Foster relationships with their team to ensure the CareHarmony culture is positive and supportive. May be asked to assist in conducting health assessments telephonically with patients Role may require a lot of time spent on the phone Other duties as applies

Required Education and Experience: Medical Assistant certificate must be active and in good standing

Physical Requirements This position is sedentary and will require sitting for long periods of time This position will require the ability to speak clearly and listen attentively, often by telephone, for an extended period of time The position will require the ability to understand, process, and take thorough notes in real-time on telephone conversations
